Disability Services Navigator

Linn County

This position supports individuals with disabilities in accessing services and making informed choices about long-term supports. The Disability Services Navigator (DSN) utilizes the Science of Hope and person-centered planning to deliver services such as Information and Assistance, Options Counseling, (Home and Community-Based Services (HCBS) System Navigation, and Service Coordination of Long-Term Services and Supports (LTSS).

Disability Services Navigators (DSNs) are anticipated to be based in Buchanan, Dubuque, Johnson, Linn, and Scott Counties.

MAIN ESSENTIAL DUTIES:

  • Conducts intakes and completes eligibility documentation for services, including assessments and social histories
  • Provides current information and assistance on available opportunities and services
  • Drafts funding requests and monitors service authorizations and needs
  • Determines initial and ongoing eligibility, maintaining records and making referrals as needed
  • Supports clients in accessing identified services to maximize self-determination

MINIMUM QUALIFICATIONS:

  • Bachelor’s degree with at least 30 semester hours in a human services field
  • 1 year of experience delivering services to individuals with disabilities

HOURLY WAGE:

  • New Hire: $26.09/$27.13
  •  90 days: $28.49 
  • 1 year: $29.99  
  • 2 years: $31.63 
  • 3 years: $33.29

*Wage effective 7/1/25

SHIFT:      

  • Monday – Friday | 8 a.m. – 4:30 p.m.
